As part of our school policy and in order to provide the safest possible environment for your child, we are required to keep medical information up to date.
If your child has a medical condition which we should be alerted to, we ask that a Medical Record Form is completed and supplied to the school office.
Some conditions, such as Asthma and Anaphylaxis, require an Action Plan to be completed by your medical practitioner. If your child has been diagnosed with such a condition, please ensure that a current Action Plan is provided with the Medical Record Form. Action Plans are required to be updated each year.
Action Plan please bring the required information and any medication to the school office on the first day of school in order for your child’s needs to be met appropriately.
